Overview
The American Heart Association’s primary objective is to lower the incidence of cardiovascular disease. While the rate of this disease has decreased significantly over the past ten years due to advances in therapeutic techniques, outcomes for those with cardiovascular illness stand to advance dramatically with effective home monitoring and tracking of cardiovascular risk factors.
Challenge
To attack this challenge, the AHA sought to transform its Blood Pressure Management Center into an expanded patient care portal where users could track their risk data, get personalized recommendations and feedback based on their information and communicate with their provider.
Solution
Get Real Consulting worked with the AHA to design, develop and deploy a completely online product, www.Heart360.org. Built to take advantage of Microsoft’s HealthVault capabilities, Heart360 is a fully realized online cardiovascular wellness center where users can record health information such as blood pressure, blood glucose, weight, cholesterol, and more, either through the web or from a device directly connected to their computer. They can view a history of their information, as well as charts and reports, which they can download and send to their provider. The system monitors the information they enter and provides them with personalized content, pulled from the AHA’s existing store of online content, which is directly relevant to their health condition based on their entries. Future versions of the product will provide powerful tools to help providers manage and monitor the health information from their patients as well as expanded communication functionality to better link patients, caretakers and providers.
Benefits
Heart360 is giving patients and caretakers a powerful online tool that promises to improve outcomes for those with cardiovascular disease or risk factors. The American Heart Association will be testing the tool during clinical trials throughout the next year.
